In this study, the authors examined the species of Campylobacter recovered from the extraintestinal sites of patients with Crohn's disease and ulcerative colitis. During a 4-year period, bacterial cultures were obtained from perianal lesions of eight Crohn's disease patients (six patients in active disease and two patients in remission), from skin lesions of four patients with ulcerative colitis, and from gastric juice from six patients with ulcerative colitis. Stool specimens were obtained from 12 control patients, and from one of the patients with Crohn's disease, from whom three Campylobacter jejuni isolates were previously obtained. Of the 31 Campylobacter species isolated from patients with Crohn's disease, 29 (93%) were Campylobacter jejuni, and 1 was Campylobacter coli. The species isolated from the Crohn's disease patients with stools positive for Campylobacter jejuni were C. jejuni (26 of 26, or 96%) and C. coli (1 of 26, or 4%). C. jejuni was the only Campylobacter species isolated from two Crohn's disease patients (1 of 2, 50%) whose stools were negative for Campylobacter jejuni; both were isolated from skin lesions. Only one Campylobacter species, C. coli, was isolated from the control patients; this isolate was isolated from a patient with ulcerative colitis. Of the 31 Campylobacter species isolated from patients with ulcerative colitis, 30 (97%) were C. jejuni, and 1 was C. coli. C. jejuni was the only Campylobacter species isolated from two patients (1 of 2, 50%) whose stools were negative for Campylobacter jejuni; both were isolated from gastric juice.
